What we collect

When you submit the application form we collect the information you provide: name, phone number, email address, city/state, licensing status, and anything you write in the free-text fields. We also record the approximate country of the request and basic technical context needed to operate the site.

How we use it

• To contact you about your application by phone, text, or email — that's the entire purpose of the form.
• To send you a short series of emails about the Maxivita opportunity. Every email includes an unsubscribe option.
• To understand where applicants come from (referral source).

What we don't do

• We do not sell your information.
• We do not share it with third parties except the service providers that run this site and deliver our email (Cloudflare, Resend).
• We do not run third-party advertising trackers on this site.

Retention & your choices

Application records are kept while relevant to recruiting. To have your information corrected or deleted, or to opt out of contact, email [email protected] and we'll take care of it.
